The situation seems to fluctuate in Porvenir, however there is still only an Immigration office here, and no Port Captain. This means that it's possible to clear into the country with Immigration, however, a Panama cruising permit will have to be obtained later at Puerto Lindo (50nm from Porvenir) or Colon/Shelter Bay, or a manual permit obtained from the Port Captain in Puerto Obaldia if coming from Colombia.
If clearing out of Panama at Porvenir, first clear out with Immigration, but you must then clear out and get an international zarpe in Puerto Lindo or Colon (or elsewhere) prior to departure.
In Porvenir, the Harbor Authority, Immigration and Guna Congresso are all based in the same building (three small white housed behind the pier with "General Guna Congresso" on the front door), making paperwork and payment of fees very easy.

Possibly the current simplest solution to clearing into Panama for cruising the San Blas is to go to Linton (Puerto Lindo) to get the cruising permit (Port Captain open Mon-Fri 08:00-17:00), then take a bus (less than $2), or taxi, to Portobelo half an hour away, for Immigration to stamp your passports. You can then sail to San Blas and get the Kuna permit at Porvenir (approx. 8 hour sail).
In the San Blas Islands you will need to pay a local tax to the Guna Yala Congresso per crew member, plus an anchoring fee. See Panama Fees for the latest.
It is likely that a local tax will be charged in each populated anchorage, but will probably include a guided tour ashore.
Fees to depart the San Blas are $4.20 + $1.50 clearance. At weekends or holidays there is a $20 overtime fee for consent to depart.
Beware of locals posing as agents charging very high prices to assist with departure clearance.
