Need information on medical service opportunities

My wife and I will be leaving Seattle in June 2007 for an extended cruise that will include the west coast of Mexico and Central America, followed by Polynesia , New Zealand, continuing west via South Africa and the Caribbean. I am a Physical Therapist and Cindy is a Family Nurse Practitioner. We wish to return the wonderful favors that life has given to us by providing medical service as needed to local residents as we travel. Payment would be accepted, but is not required (room and board would help if the location is away from the boat). Clinics, hospitals ???? We don't want to reinvent the wheel. as for sure someone out there has done what we want to do. We appreciate any suggestions and directions.
